If you complete the consultation request form, we collect the details you enter: your first and last name, phone number, email address, preferred contact method, and optionally the service you are interested in. We use that information for one purpose — to contact you about scheduling.
Our web host also processes standard technical information that every website receives, such as your IP address, browser type and the pages you requested. This is used for security and to keep the site running.
What this website does not collect
The forms on this site deliberately do not ask for diagnoses, medications, medical history, symptom details, laboratory results, Social Security numbers or insurance information. Please do not submit that information through this website. Clinical information is collected during your consultation on an appropriate platform.
